Healthcare Team

District Nurses

Our district nurse team work closely with the doctors and practice nurses. They visit patients at home, play a particularly important role in the care of patients who are seriously or terminally ill, deal with dressings, injections, the removal of stitches after operations in hospital, and run some clinic sessions.

Out of Hospital Team

This team includes community matrons, occupational health, physiotherapy and falls prevention etc.

Should you require assistance by this team, your GP will refer you to them and they will organise care that suits you and your circumstances. This team will keep your GP up to date with your progress throughout your care.
